Coordinated care
A programme in which your family doctor manages your long-term condition throughout your care, working with a specialist without sending you to join a specialist clinic waiting list. At Teledoctor, we offer one pathway: respiratory care for bronchial asthma and chronic obstructive pulmonary disease (COPD).
Who it is for
- you have been diagnosed with bronchial asthma,
- you have been diagnosed with COPD,
- or your doctor suspects one of these conditions and wants to confirm it with a test.
How to get started
- Call reception or speak to your doctor at your next appointment.
- We will arrange a comprehensive consultation, which is longer than a standard appointment.
- After your tests, you receive a care plan and scheduled appointments.
What changes
Without the programme, the process usually looked like this: you received a referral to a respiratory clinic, called to book, were offered an appointment several months away and had to manage your medication on your own in the meantime. In this programme, you stay with us — tests, specialist advice and your treatment plan are arranged through the clinic, with someone making sure the next step actually happens.
Comprehensive consultation and care plan
It starts with a longer appointment. The doctor reviews your full medical history, checks how you are managing your medication, orders tests and writes an Individual Medical Care Plan for you to take home. It sets out what should happen and when: which tests you need, your next review and what to do if your condition worsens.
Tests
The main test in the respiratory pathway is spirometry, including bronchodilator reversibility testing. It determines whether the condition is asthma or COPD and how it responds to treatment. The doctor decides which other tests are needed at the comprehensive consultation.
Respiratory specialist consultation
If your case needs a specialist’s opinion, your family doctor consults a respiratory specialist within the programme — sometimes at a joint appointment, sometimes in a discussion between doctors. You do not need to obtain a specialist clinic referral or join a waiting list.
Education and dietary advice
This is the part patients most often overlook, yet it can make the biggest difference in lung disease. We check that you are using your inhaler correctly — often medication does not work because it fails to reach the right place, not because it is the wrong medicine. We also discuss things at home and at work that make symptoms worse and, if you smoke, how to stop. The programme also includes dietary advice.
Care coordinator
The programme includes a person responsible for keeping your appointments on track: booking tests, reminding you about visits and putting you in touch with your doctor when something is not right. This is who you call if you lose track of your plan.
The entire programme is free — the comprehensive consultation, tests, specialist advice, education and dietary consultation are funded by the National Health Fund (NFZ).
