I need a sick leave certificate
An electronic sick leave certificate after a telephone or in-person consultation. We issue certificates after assessing the patient — in person, or by phone if the symptoms allow this. We do not issue sick leave certificates without a consultation with a doctor.
How it works
- Call reception and ask for an urgent consultation.
- Reception arranges a same-day telephone consultation or an in-person appointment.
- The doctor assesses your health and issues an electronic sick leave certificate if there are grounds to do so.
- The electronic sick leave certificate is sent to Poland’s Social Insurance Institution (ZUS) and your employer electronically. You do not need to deliver any documents.
What to have ready
- your PESEL number — and your child’s PESEL number if requesting leave to care for your child;
- your employer’s NIP tax identification number, if this is your first sick leave certificate with that employer;
- information about when your symptoms started.
